My First Clinical Experience

This essay is designed to reflect on my experience while on clinical practice in a hospital setting as a student nurse and is based upon an event I observed. I will be discussing my feelings and thoughts about the event and analyzing the theory associating with relevant evidence for practice. Also, evaluating my understanding of nursing practices and skills, and to demonstrate the application of core components of the NMC Code of Conduct, (NMC, 2018). I will be considering person-centered care and whether this was adhered to at the time of the event.

I will be applying relevant models taking into consideration the legal and ethical factors that may impact patient care.

The NMC Code states that nurses should practice in accordance with The Code, using the ethical and legal framework to ensure patient well-being and respect confidentiality. The reflection will be based upon patient confidentiality and I will be using this event for my reflection and refer to the patient as “Mr.

Johnson”. In this essay, confidentiality will be maintained using pseudonyms. It is a key part of NHS codes and contracts of employment that patient confidentiality is maintained, and the information is only disclosed with the patient’s permission, (Nursing.Times, 2020). The Data Protection Act of 2018 also advises that we follow strict rules when using personal information. (GOV.UK, 2018)

This essay will be reflected in the medication administration I have observed during clinical practice in a medical ward. This reflective essay firstly provides a short analytical consideration of the nature and importance of reflection.

The reflection is based on the Driscoll model (2007) developed by John Driscoll to provide the framework for the reflective essay. The model is based on Terry Barton’s (1970) 3 stem questions; What, So What (the feelings about the experience), and Now What (if it happens again what would you do differently?). (Cambridge, 2020). The use of this reflection will allow me to explore different areas to evaluate and improve my experience in nursing practice. This will benefit me in developing the necessary skills that will improve the quality of care of the patients. The reflection on the experiences will also help me improve my knowledge and understanding of the medical practices and subsequently allow self-critique. Medication administration is a big part of a nurse’s role and is considered carefully to avoid any errors. Every day some 7,000 doses of medication are administered in a typical NHS hospital (Emerald Pushing Limited, 2020), with thousands more self-administered by patients themselves in their own homes.

Medicines are prescribed by the Doctor on the ward and dispensed by a pharmacist, but the responsibility lies with the nurses administering the medication and will be accountable for any decision made. Being accountable means it is every nurses’ duty to be personally answerable for their actions and omissions during practice and must be able to explain the decision made and should be able to answer the consequences of their actions, (Royal college of nursing, 2016). As a student nurse, it forms part of your duty to be able to practice and be competent in administering medication. To administer medication nurses are required to familiarise themselves with the Five R’s which include; right medication, right route, right time, right dose, and right patient, (WHO, n.d). To achieve this, the legal framework brings together four distinct areas of accountability to protect patients from the harmful effects of medicines though allowing them to benefit from their therapeutic properties. To administer medication it involves preparation, checking, and administering the medication to the right individual.
