It forms the basis of practice. Applying the argument into the areas of a Family Nurse Practitioner, organizing foundations that will help appraise care, foster interventions, and gauge the worth of care (Aura, 2015). Models are build-up of a thesis that assist in expanding nurses' qualities and awareness, which guide their responses and set objectives.

It reflects the values and needs of the client and shapes the rule of the institution. The advancement of these tools supports the rapport between the client and the caregiver. Through the nurse's active partnership, the patient, and caregiver satisfaction with care enhanced, which will further increase the worth of care (Duffy, 2016). The chosen care model that I will examine and apply in the NP track is Person-Centered Nursing (PCN) Framework. I will discuss the selected model and its connection with the nursing metaparadigm, how I can utilize the PCN model in NP role, and advanced practice nursing and end with a summary of what I have learned in this paper.

Overview of the Selected Model

McCormack and McCance developed the PCN model in 2006, and it emphasizes keeping the patient at the center of the care.

The PCN structure has four different ideas. They are prerequisites, characteristics of the nurse taking care of the person. The care environment targets the settings in which the care is taking place. A person-centered process examines how the care is rendered. The caregivers acknowledge the clients believe, work with their choices, strengthen decision making, and consider each client as a whole person. Outcomes are the results of productive nursing, such as excellent client care experience, fulfillment with care, personal engagement, a sense of well- being and soothing atmosphere (McCormack & McCance, 2016). The PCN framework correlates with each idea of meta paradigm, including person, health, nursing, and environment. The requirements of PCN belong to nursing. It looks at whether the nurse is competent, dedicated, compassionate, knows his or herself, and aware of one's assumptions and values when giving care. The care environment points out the meta paradigm of the environment. The situations are based on client demands, staff relations, safety, and the system that promotes combined decision-making and the possibility of taking new risks (McCormack & McCance, 2016). The holistic care of the PCN framework is related to health, which aims to treat all domains of the client (Bender, 2018). Person-centered outcome refers to the meta paradigm of a person because the PCN targets the client during the entire care process.

Appraisal of the Model as a Nursing Theory

The PCN care model is helpful and improves discipline by giving a structure. It qualifies as a theory and allows the nursing to expand through analysis, science, and practice. PCN directs how this theory can be executed in the workplace. The PCN is useful in providing a structure to asses and augments the growth towards person-centered nursing. The model introduces different interventions and steps to develop new healthcare policies and practices. It is a powerful tool that helps to achieve quality and holistic care in multiple healthcare areas. The PCN supports both providers and clients to experience good relations, appreciative care, productive conversation, empowerment, and treat the family as a unit of care. Thus, the PCN has high value in nursing, and both clients and care providers will get the benefit when all parts of the model are in place (Santana et al., 2018).

The PCN can assist the NP's to observe the critical elements, know the traits, boost clinical skills, find out the obstacles to change, and targets the growth of new developments in their specialties. It empowers the NP's to explore person-centered care in their field (Santana et al., 2018). Through this model, NP inspects workplace cultures and shapes the areas that require changes (Slater, McCance, & McCormack, 2015). The PCN care model will help NP to recognize unique client perspectives. It makes the care team conscious about the relevance of including clients and supports making clinical decisions in their care. The use of the PCN model and the client's adherence to care results in reduced nosocomial infections decreased fatality rates, and period of stay in the hospital (Santana et al., 2018).

Application of the model to Advanced Practice Nursing

The PCN model is a part of NP's daily routines and can employ in various patient situations. The PCN model helps the NP to incorporate values, principles, and ideas into their clinical areas, and advance their work at a system level. By utilizing the PCN, an NP will be able to learn the cultural values and believes of the client related to their disease condition and how these impacts the care they provide. NP can promote a curative, safe work environment and give personalized care through building relations that enhance fulfillment and adherence with their regimen

According to Flagg (2015), the participation of the client and their families, and a supportive physical work environment play an essential role in the successful operation of person-centered care. An NP must uphold the practice of PCN framework as a leader and provider. NP working in an acute care setting as a provider received an elderly patient who was actively bleeding from colostomy and rectum secondary to metastatic colorectal cancer. The patient was unstable and required to resuscitate per Massive Transfusion Protocol (MTP). The family members arrived at the unit with the client, and they were upset from the sought of event. The patient and family require emotional support. The NP can target patient care by educating the families. The NP must co-ordinate the client care plan at the beginning and making sure that the client's dignity and wishes are met. The NP would invite the family to the room during the resuscitation and let them see the effort being placed towards the patient, even if it is not a pleasant situation. After stabilizing the client, NP must consult with an interventional radiologist for embolization, and collectively work with them to provide the best care. The NP can invite the family and explain the prognosis, the treatment choices, and allow them to ask questions freely. They make them feel valued, respected and treated just special as the client is unique, different from others.

Conclusion

The thesis provides a structure of one's considerations and impressions in practice. It helps NP to offer personalized care that reflects the values and demands of the client. The PCN is a beneficial tool for advanced nursing practice. It guides caregivers to give client-centered care. PCN uses in many institutions to foster the worth of care and outcomes for clients and facilities. I have learned how does the meta paradigm linked to the PCN. I realized that the goal of healthcare is to place the client at the center, which makes them feel respected and valued.
