The major propose of this reflection is to discuss the scope, nature and assessment of the given client on the interview. This assessment covers the reflection of the given client including all the aspects like appearance, mood, speech relating to mental state examination. In this reflection, I will explain the patient mental condition and using clinical reasoning cycle.

The woman is 38-year-old who is visiting GP because of her consistent low mood in recommendation of her sister. She is divorced and has 2 children; her occupation is working in a local supermarket.

She is getting persistent low mood since few months. She looks a bit untidy with not putting up the buttons on her sweater. Her hair looks messy. She is communicating without eye contact with the GP.

She is coordinating with GP by answering all the questions.

She seems to be anxious and her facial expression is fearful while speaking. Also, she has got unusual posture of moving hands while communicating with the GP. She explain that every morning she wakes up ,everything seems black .This shows that she is being pessimist with everything in life .Also ,she is bursting up into tears even upon slightest mistakes like dropping sugar .Despite ,going to work occasionally she is just spending time on her sofa demonstrates that she doesn't have any excitement or motto in her life. She has also got the feeling of being problem to everybody.

The major problem of Alison at this stage of life is being pessimist and stressing lot.

Also, being a single mother and being pay cutbacks, she is suffering from financial stress too. She is having difficulty to sleep, and her sleeping pattern is also being disturbed. She has no appetite to eat anything. Furthermore, she has also tried to harm herself by having 12 Panadol at one time. She is losing weight too from the last 2 months. Also, she is stressed about the death of her father. As, her boyfriend, Dave is not calling or texting her like before she is tense that he is being exhausted and tired of her. Alison is feeling down, exhausted and getting problems on concentration.

To eradicate or to solve all the above-mentioned problems Alison is experiencing, relevant and specific goals needs to be created. Goal is defined as ambition toward some specific problem which would result in measurable, attainable and relevant positive result. The primary goal for Alison is to make her see positive side of her life and reduction on her stress level. As per Clinical reasoning cycle, another step needed on reflection of Alison is to take the action for the problems she is facing on her life. The first Action that can help Alison is to encourage her to spend and enjoy her time with children as they are the ones, she loves a lot. This can help her to appreciate and enjoy her life that can reduce her isolation. In Addition, Education should be prioritized for the actions to be taken when she gets the feeling of harming herself or to other people. This can be calling emergency helpline or her good friends, her sister or her boyfriend, Dave. She should also be encouraged to spend the time with people who can make her feeling love, positive and valuable. Moreover, being a single mother and financially stressed she should be motivated to work on regular basis.

After applying all the above-mentioned goals and actions according to Clinical Reasoning Cycle another step is Evaluating the outcomes, as it helps to recognize if the goal and action is working or not. The outcome of her mental state needs to be assessed, if it works the actions should be continued. However, If the actions mentioned does not work, the other Goals and actions suitable for Alison needs to be created.
