A lady that I used to support was told that her father had passed away. One week later her mother also passed away to. As imagined the lady was emotionally distraught about her bereavements, not only had she lost one parent she had lost to. The lady did not have any other family members that she saw regularly so she felt like she had no one now. Emotional support and comfort was given to her whenever she needed it. As it was close to Christmas she felt it would be nice to visit a church and leave some flowers. The ladies beliefs were taken into consideration at this time and she received support to visit a local church to do this. The lady found great comfort in doing this. Over the course of the next view months occasions such as her parents birthday, mother’s day and father’s day came up.

The lady would also like to make things such as cards and flowers and would usually give them to her parents. As she couldn’t do this anymore and these values meant a lot to her, she received support again to fulfill her wishes and took these items to the local church. Throughout this time she always received respect and sympathy. The lady then decided that she would like to move to another service, one where she could have her own space in a self-contained flat. As this was her wish, we acted upon it to ensure that we could communicate with the relevant people to help her move on her behalf. The lady was included with decision making throughout the process. The lady did have a brother that we contacted to ensure that we could make her family aware of her decision. The lady then moved within a month. Throughout this sensitive and complex situation I was able to ensure the ladies wellbeing was ok. Promote the lady to be included in decision making. I was able to be respecting of her values and beliefs.

Explain how finding out about an individual’s history, preferences, wishes and needs is an important part of creating a good care or support plan. By finding out the individual’s history it will allow others to have a better understanding of that person. The individual’s preferences being in a care plan is very important for you to be able deliver support. If you did something that an individual did not like you may cause them distress. If you know how to support an individual if they were feeling upset you may be able to help them more with sensitive issues. The needs and wishes of each individual will be very different to someone else. By knowing this information it will inform others who deliver support to, it will enable accuracy and relevance when developing and implementing an individual support plan. Biii – Explain why it is important to review care or support plans with an individual, and to monitor their changing needs or preferences. Individual’s lives change from day to day. Individual’s relationships may change with friends and families. They may develop new relationships that need to be in their care plan.

Medical needs may change, such as medical conditions and medication changes. The individual’s abilities may change which could have an impacted on the support that is needed. Individual’s daily and weekly activities that they take part it may change. It is important that everyone is informed of this. To ensure that the best quality of care is provided changes and preferences need to monitored regularly and updated in their care plan. Biv Explain how useful care or support plans can be in supporting person centered practice If you have been supporting an individual for a while you would have gotten to know them fairly well. You will be able to assess the accuracy of their care plan and she if the information provided in relevant. You will be able to know the individuals preferences and will be able to identify if they are different from what is stated in their care plan. If there care plan needs updating to reflect that person you will be able to discuss it with your colleagues and see if they also feel the same.

Task C – Case studies
Case Study 1

Ci – Some individuals may not have the ability to express their views. This may mean that sometimes we are able to obtain consent from an individual receiving support. There are various reasons as to why this may be. The individual may have sensory, visual or verbal impairments. There may be a language barrier. Some individuals may have a lack of awareness of the choices offered and may not know the implication of their choices. Some individuals may be suffering from confusion or mental illness. Cii – There are various different ways you may be able to gain consent to activities or actions. By ensuring the individual has access to information surrounding activities and action and be allowing them to have alternative decisions. This may help them understand the choices offered. Allowing the individual to have time to decided and think about the implications of their choices.

You could try different communication techniques such as a language interpreter, using active listening, making involving another person who is able to communicate with the individual to enable a two-way discussion. You could write it down on some paper. If you still aren’t able to obtain consent you could seek further advice. Ciii- Explain what a social worker can do if they cannot gain consent or is unsure of the response. If action has started to be taken it should be ceased as you have not be able to gain consent. You should remain calm and empathetic to ensure you are not putting the individual under any stress. You should consult your manager to see if any feedback or advice can be given. Everything that you have done regarding the situation must be documented.

Case Study 2

C –
Some people may try and impose their view on an individual. There are various different approaches you can use to help individuals make informed choices. You should provide them with all relevant accurate information to help them make their choice. When discussing choices with them avoid making your own assumptions and do not let your own prejudice influence their choice. Enough time should be given to allow them to think about their decision to ensure they are happy with it. You could ask their next of kin to provide encourage and deliver advice to the individual. You could ask gather information from an expert.

Sometimes decisions may be made by others for the individuals, there are ways in which you can help them challenge the decision. You could support and encourage the individual to speak with confidence and voice their opinions. If you, as a care worker are concerned about decisions that have been made, you could speak to a senior member of staff. If the individual has confided in you that they are not happy with what decisions have been made, you could offer to voice concerns on their behalf. If something has been non –compliant with legislation requirements or policies, they may be unaware of this, so you could then help them identify this.

Sometimes the views of others such as, medical practitioners, social care workers and family members are allowed to influence the choices individuals make. There are possible consequences that may occur from this. The individual may have feelings of alienation as they feel they do not have any say in their life. This may lead to stress and anxiety and low self-esteem. If a poor choice has been made by someone else for example surrounding someone’s physical wellbeing it could result in discomfort or pain. There could be complaints from the next of kin.

Task D Short Answer Questions
Di – Explain what is meant by “active participation”.

Active participation means inclusion, self-empowerment, communication, feeling valued and being part of the solution. Dii – Explain how active participation can support all aspects of an individual’s preference and needs. Active participation allows the individuals to choose what they want in their life. It allows them to choose what activities to participate in. This ensures they are feeling empowered. Social interaction is being met. Diii – Explain how everyone involved in an activity or an event such as a review understands how to apply the principles of active participation. You could explain the use of discussions so everyone can voice their ideas and concerns. Providing motivation to the individual will build their confidence. The benefits of active participation could be provided prior to the review to raise awareness. Div –. Give two ways that a risk assessment can support the right of an individual using the service to take risks and make choices Risk assessments can identify the potential danger and harm that might occur if the service user takes part in a activity as well as identifying the benefits it will have the service user.

The risk assessment is put in place to ensure the service user can take part it their chosen activity, the harm and danger that may occur is then identified and then the ways in which this can be reduced. Once this has been established it will be the service user will be able to do what they have chosen in the most safe way possible. Dv- Explain how the responsibilities of all concerned with the care of an individual can be supported by a risk assessment. When the dangers and hazards have been established the way in which to prevent this will usually involve responsibility of all concerned with the care of the individual.

The risk assessment will establish the best way the individual needs to be supported to ensure the person is safe. Dvi – Describe two different uses of a risk assessment in adult social care settings. Risk assessments can be used to prevent accidents happening by identifying accidents that may occur before they have happened. Risk assessments also allow the people supporting the individual to be able to follow certain procedures or steps that have been put in place in the event of an accident occurring.
