After that incident happened, I encounter nursing ethical dilemmas in a situation which I cannot do what I consider the right thing for the patient and just be a witness for unfair treatment that can impact patient health and can cause patient loss their thrust on health care works. However, surgical site infections account for 14% to 17% of all hospital-acquired infections and 38% of nosocomial infections in surgical patient’s. It is stated by Spagnolo, Ottria, Amicizia, Perdelli, and Cristina in 2013. Nurses play an important role in the prevention and control of surgical site infections (SSIs) because they undertake a high proportion of the treatment and care of surgical patients.
Patel (2015) states that a sterile field must be established and maintained to reduce the risk of contaminating the surgical or procedure site. The sterile field is created by placing sterile towels or drapes around the surgical site Additional sterile fields may also be established, such as on the stand that will hold instruments and other items that are needed during the procedure. Even though it is everyone’s responsibility to preventing infections in the OT, but we as a scrub nurse must be aware when there is a break in sterile technique and there must be a way for how to address them and act to correct them.
As asserted by Leodoro, Labrague, Dolores, Arteche, Begonia, and Nenita (2012) sterile technique plays a vital role in the control and prevention of SSIs and surgical site contamination. Looking back on this incident I felt guilty that I did not act to challenge the staff nurse practice before she continues to prepare the procedure. I should take immediate action and corrective as soon as possible after a break of sterility is realized. Besides that, I am not yet confident to teach and correct the staff nurse since my status as a student. Here I also felt I had failed in my duty of care as a healthcare provider.
Throughout this incident, I felt that several valuable issues had been highlighted. Firstly, I realized the importance of acting assertive with co-workers to enhance the safeguard of our patients. In fact, I feel ashamed that I did not speak up and stay silent when seeing the standards of practice in sterile technique were being broken or malpractice occurs, and it can harm the patient likes SSIs. From this incident, I was found a few negative issues, but it still showed me some positive concern to make me awake in the future to take care of my patients, need. For example, the way staff nurse approaches the patient with gentle even though the patient is a small boy, but she still gave respect and speak politely to the patient. This shows that attitude should be followed and practiced by all the hospital staff especially the nurses all the times so that patient’s dignity will be safe.
Beyond that, the incident makes me think as the perioperative nurse must sense that health care worker should take a minute to consider your special role as a caregiver and sincere desire to do the right things to your patients. Asepsis technique should not be compromised for the sake of convenience of the caregiver and I always keep that in my mind all the time.
