There is a probability that many people in later life get at least one chronic disease, 65% of people over 65 years have more than one disease and this number rises over 85% for people over 85 years (Stassen et al., 2018). According to World Health Organization (WHO) chronic is a disease that persists for a long period of time. Chronic disease tends to become more common with age. (WHO, 2018). Management of long-lasting conditions is among the most presseing challenge of healthcare systems worldwide.

Electronic Health Record (EHR) is an important tool towards accomplishing or gaining with effort better healthcare using such technology, although, across the world EHR putting into use has experienced a high failure rate.

However, South Africa has made a related to a plan to reach a goal decision to put into use of EHR system in the public health area. The use of Electronic Health Records (EHR) in the health care industry is now supported in many countries including countries in Europe and North America.

Chronic disease management identifying and reducing disparities: care disparities can be reduced in the management of chronic disease with Electronic Health records through tracing care provided to each patient by each provider meaning appointment status, routine testing recall and recall notices can be monitored by all.

EHR system will help to improve the referal process between practitioners through streamlinung he processes and providing the referred provider with immediate access to pertinent health inormation.EMR could generate registries or shared diagnosis meaning that one has immeiate access to population in the instance of public health outbreas or knowing patients' outcomes in chronic disease management.

Collecting data in a meaningful way will help provider information regarding access to care or similar patient. Through this measure utilizing electronic health record in an interdisciplinar way will benefit all stakeholders.

2. Background of the research problem

We are in a world dominated by non-communicable chronic disease according to (Stassen ,2018). over 70 percent of death in this world are due to these chronic diseases. It was argued that addressing chronic disease is a major challenge for healthcare systems around the world, which have largely developed to deal with acute episodic care, rather than to provide organized care for people with long term conditions (Reynoldset et al., 2017).

For South Africans leaving with chronic conditions keeping track of all their medications, specialist reports, consult x-rays and scans is a full-time job, but with a comprehensive electronic health record, patient information is brought tcogether in one place making it easier for doctors and healthcare team to work with patients to better manage chronic conditions together. All the patient's records i.e., Immunization record, lab reports, treatment are right there. It actually makes it far more efficient. They have hypertension reminders that will come out that it is time to recheck their blood work all those reminders are listed there for the practitioners to action every time the patient comes in.

In Canada, the primary health care makes use of the EHR to keep track of patients' information. This can lead to an increase complication and difficult dicision maing with regards to care. In order to properly address these patients' collaborative team-based health care is needed to meet the incease burden of chronic disease with the aging population. 3. Statement of research problem

Healthcare workers know a lot about how to prevent or treat some of the diseases but they do not know how to get them into practice. The cost of inaction is an unsustainable health care system for countries around the world. It is people continuing to face chronic diseases which either could have been prevented or it can be much better addressed than currently.
